In 2011, the Global Initiative for Chronic Obstructive Lung Disease (GOLD) released a consensus report, Global Strategy for the Diagnosis, Management, and Prevention of COPD. It recommended a major revision in the management strategy for COPD that was presented in the original 2001 document. Updated reports released in January 2013, January 2014, January 2015, and December 2015 are based on scientific literature published since the completion of the 2011 document but maintain the same treatment paradigm. Assessment of COPD is based on the patient’s level of symptoms, future risk of exacerbations, the severity of the spirometric abnormality, and the identification of comorbidities. The January 2015 update added an Appendix on Asthma COPD Overlap Syndrome, material prepared jointly by the GOLD and GINA Science Committees.
The GOLD report is presented as a “strategy document” for health care professionals to use as a tool to implement effective management programs based on available health care systems. The quadrant management strategy tool is designed to be used in any clinical setting; it draws together a measure of the impact of the patient’s symptoms and an assessment of the patient’s risk of having a serious adverse health event in the future. Many studies have assessed the utility/relevance of this new tool. Evidence will continue to be evaluated by the GOLD committees and management strategy recommendations modified as required.
GOLD has been fortunate to have a network of international distinguished health professionals from multiple disciplines. Many of these experts have initiated investigations of the causes and prevalence of COPD in their countries, and have developed innovative approaches for the dissemination and implementation of the GOLD management strategy. The GOLD initiative will continue to work with National Leaders and other interested health care professionals to bring COPD to the attention of governments, public health officials, health care workers, and the general public to raise awareness of the burden of COPD and to develop programs for early detection, prevention and approaches to management.[/vc_column_text][/vc_column][vc_column column_padding="no-extra-padding" column_padding_position="all" background_color_opacity="1" background_hover_color_opacity="1" width="1/3"][vc_column_text]

Przewlekła obturacyjna choroba płuc (POChP) wciąż stanowi poważny problem zdrowia publicznego. Wśród przyczyn przewlekłej chorobowości i umieralności w Stanach Zjednoczonych POChP zajmuje 4. miejsce, a na całym świecie, według badania opublikowanego przez Bank Światowy i Światową Organizację Zdrowia, w 2020 roku zajmie 5. miejsce. Mimo że w ostatnich latach POChP przyciąga coraz większą uwagę społeczności medycznej, to nadal jest ona mało znana w całym społeczeństwie, a instytucje odpowiedzialne za opiekę zdrowotną i urzędnicy państwowi nie poświęcają jej należytej uwagi.
W 1998 roku w celu zwrócenia uwagi na POChP, jej leczenie i zapobieganie – zaangażowa- na grupa naukowców zachęciła amerykański National Heart, Lung, and Blood Institute oraz Światową Organizację Zdrowia do utworzenia Światowej Inicjatywy Zwalczania Przewlekłej Obturacyjnej Choroby Płuc (Global Initiative for Chronic Obstructive Lung Disease – GOLD). Do ważnych celów GOLD należą: zwiększenie w społeczeństwie świadomości na temat POChP oraz pomoc milionom osób cierpiących na POChP i umierających przedwcześnie z powodu tej choroby lub jej powikłań.
Pierwszym etapem inicjatywy GOLD było przygotowanie uzgodnionego raportu zatytułowanego „Światowa strategia rozpoznawania, leczenia i prewencji POChP” („Global strategy for the diagnosis, management, and prevention of COPD”), opublikowanego w 2001 roku. Raport przygotował zespół ekspertów pod przewodnictwem profesora Romaina Pauwelsa z Belgii, złożony z wybitnych specjalistów w dziedzinie pulmonologii, epidemiologii, ekonomii społecznej, zdrowia publicznego i edukacji zdrowotnej. Zespół ten, opracowując wspólny dokument, przeanalizował istniejące wytyczne dotyczące POChP i nowe informacje na temat mechanizmów patogenetycznych tej choroby. Niniejszy dokument ma taką samą formę jak pierwotny raport, ale został zaktualizowany na podstawie licznych publikacji na temat POChP, jakie się ukazały od 2001 roku.
Po opublikowaniu pierwszego raportu w 2001 roku utworzono sieć międzynarodowych ekspertów – Krajowych Liderów GOLD, w celu wdrażania zawartych w nim zaleceń. Wielu z tych ekspertów zainicjowało badania nad przyczynami i epidemiologią POChP w swoich krajach i opracowało innowacyjne strategie rozpowszechniania i implementacji wytycznych postępowania w POChP.
Doceniamy ogromną pracę, jaką Krajowi Liderzy GOLD wykonali dla swoich pacjentów z POChP. Mimo postępów osiągniętych przez 5 lat, jakie upłynęły od publikacji pierwszego raportu GOLD, mamy jeszcze wszyscy wiele do zrobienia, jeśli chcemy opanować ten ważny problem zdrowia publicznego. Działalność GOLD będzie kontynuowana, aby zwracać na POChP uwagę rządów, urzędników odpowiedzialnych za zdrowie publiczne, pracowników opieki zdrowotnej i całej opinii publicznej, ale będzie konieczny skoordynowany wysiłek wszystkich osób zaangażowanych w ochronę zdrowia.
Chciałabym wyrazić podziękowanie za pracę członkom Komitetu Naukowego GOLD, którzy przygotowali ten nowy raport. Liczymy na dalszą współpracę z zainteresowanymi organizacjami i Krajowymi Liderami GOLD, aby osiągnąć cele tej inicjatywy.
